INTERxVENTOn Call is perfect for busy people who want the personal attention of health professionals at times that fit their schedule.  Participants communicate with our health professionals, called Mentors, via telephone or our web site.  INTERxVENTOn Call integrates exercise, nutrition, weight management, stress management, and other important preventive strategies to help you optimize your health and total well-being.  INTERxVENTOn Call also has options to help you prevent or manage chronic diseases, such as heart disease, stroke, high blood pressure, high cholesterol, high triglycerides, diabetes, and obesity.  Approximately 20 counseling sessions are scheduled over a one-year period.  Your mentor helps you stay motivated and on track to achieve your health and lifestyle goals.  Your individualized plan addresses your specific goals, preferences, and circumstances.  INTERxVENTOn Call introduces one concept at a time in an easy-to-understand and carefully sequenced way to help you master important lifestyle skills.  People who have participated in INTERxVENTOn Call are as successful as those who have face-to-face visits with mentors.  See “Meet Our Mentors” to get acquainted with some our outstanding staff of dedicated professionals. 

Your participation in INTERxVENTOn Call  includes:

·        An initial assessment of your past medical history, current health status, family medical history, readiness for change, eating and exercise habits, and much more

·        Graphic displays of your current risk factor values and realistic short-term and long-term goals based on standards established by national expert groups

·        A Personal Action Plan with specific recommendations for improving important aspects of your lifestyle

·        An Aerobic Exercise Plan for an exercise suitable for you, including an individualized target heart rate and/or exertion rating

·        An individualized Meal Plan that recommends the number of daily servings of foods from the five food groups to eat at breakfast, lunch, dinner, and for snacks

·        A target goal for the maximum number of grams of fat you should eat each day

·        Recommendations to see your doctor about new medications or medication changes, if appropriate to help reduce your risk of heart attack and stroke

·        Easy-to-understand educational kits and audios covering more than 60 specific topics and skills

·        Self-management tools to monitor your eating and exercise habits

·        On-going support, motivation, re-evaluation and revision of your Personal Action Plan

·        Follow-up evaluations after 12 weeks and one year

·        Tracking for adherence and quality assurance

Your sessions are approximately 15 minutes in duration.  Appointments are scheduled in advance.  A toll-free number is available within the United States.

Options for Receiving Materials (Choose One):  

We can mail your program materials to you or you can access them at our web site.

Through the Mail

·        We mail you a packet containing materials and instructions for your Initial Assessment.  You are responsible for costs associated with measurements and blood tests, if needed.

·        You return your personal information to us and we develop an individualized program for you. 

·        Your Personal Goals/Action Plan is returned to you by mail within two weeks, along with a binder of educational kits, diaries, logs, charts, and surveys.  Materials for two Follow-up Evaluations (after 12 weeks and one year) are included at no extra charge.  Audios are provided to guide you through your Goals/Action Plan and help you implement your program. 

  At Our Web Site

·        You complete the Initial Assessment via our web site.  You are responsible for costs associated with measurements and blood tests, if needed.  When it’s complete, you print your Personal Goals/Action Plan.

·        You access all educational kits, diaries, logs, charts, and surveys directly from the web site.  You may choose to print materials for your personal use as desired.  Audios are also available via the web site.  Your computer must be equipped for audio capabilities.

·        When it’s time for your Follow-up Evaluations, just enter your information and your Personal Progress Reports are available to you.
